 HCH Care Coordination HCH provides members with personalized care coordination and navigation assistance to help eliminate barriers to health and wellbeing. Our care coordination team members work directly with members and their primary care providers (PCPs) to identify, understand, and take control of health risks and chronic diseases so that members have the best health outcomes possible.


Telemedicine

MDLIVE
Leander ISD provides you and your eligible dependents with MDLIVE if you enroll in one of the LISD medical plans.​ MDLIVE gives you 24/7/365 access to a doctor through the convenience of a virtual visit. There is no monthly fee to join. PPO plan members pay $35 per consult; CDHP plan members pay $44 per consult. Once CDHP members reach their deductible, they will be reimbursed for 80% of the consult fee by MDLIVE (processing time takes 4-6 weeks).
